Can hair loss be a result of scalp problems?

I’ve had flaky and itchy scalp for the past 2 years, and I have I’ve noticed my hair getting thinner, could this be caused by excess scratching of the scalp? I’ve noticed when my hair falls out there is excess skin on the follicle.
